I am planning a street performing/busking trip next summer during which I will be coming to Milan in June, for two days.
Can I do street performing with my acoustic guitar on the streets of Milan, if I don't use any amplification, no microphones etc?
Do I need to get a permit for this, and if I need a permit, where can I get one in advance?

Which is yes - you have to ask for a specific permission.
You can do by addressing the relevant staff at TMTI.portaleturismo@comune.milano.it (they read this message in carbon copy).
